2015 Open Enrollment for State Health Insurance Kicks off October 1

healthAnnual Enrollment will be held Oct. 1-31, 2014. 

The choices you make during Annual Enrollment are for benefits effective Jan. 1, 2015, throughDec. 31, 2015. If you are satisfied with the plan in which you are currently enrolled, you do not need to re-enroll in that plan. However, if you are enrolled in the Enhanced 80/20 Plan or the Consumer-Directed Health Plan (CDHP) and wish to maintain or lower your premium you will need to complete the wellness premium credits described below by Oct. 31, 2014. If you do not complete the wellness activities and are enrolled in the Enhanced 80/20 or the CDHP, you will remain in that plan; however, your employee-only premium will be higher.

Wellness Premium Credits

The premium credit activities below are for members enrolled in the Enhanced 80/20 Plan and CDHP.

Smoker Attestation

The subscriber and spouse, if applicable, will need to attest to being a non-smoker or commit to a smoking cessation program. Even if you attested during last year’s Annual Enrollment, you will need to re-attest. The smoker attestation can be completed only during Annual Enrollment through their enrollment system.

Health Assessment

The subscriber will need to complete or update the Health Assessment through the Personal Health Portal. If you have taken or updated your Health Assessment between Nov. 1, 2013, and Oct. 31, 2014, through the Personal Health Portal which can be accessed at  or by phone, it will count toward premium credit. If you are not sure when you completed the Health Assessment or do not have Internet access,  call 800-817-7044. Please go ahead and update or complete your Health Assessment now. You do not need to wait until Annual Enrollment.

Primary Care Provider Selection

The subscriber will need to select a Primary Care Provider for himself/herself and any covered dependents. You may select a PCP any time prior to enrollment by logging into the enrollment system. If you already selected a PCP during the last Annual Enrollment or at some other time, you will receive the premium credit. If you have added any dependents, make sure to select one for them as well to receive the credit.

Heather Mullins is the Chief Academic Officer in Newton-Conover City Schools. She is a North Carolina Teaching Fellow who spent 12 years as a high school English Teacher. She received her B.S.Ed. in Secondary English Education from Western Carolina University. Heather completed her National Board Certification in Adolescent Young Adult English Language Arts in 2002. She holds an M.Ed. in Academically and Intellectually Gifted from UNC-Charlotte. Heather has served as a Curriculum Specialist in Hickory City Schools, an adjunct professor at Lenoir-Rhyne University, and a Professional Development Consultant for the North Carolina Department of Public Instruction. She completed her Ed.D. in Educational Leadership through Western Carolina University. Heather is one of the co-founders of #NCed Chat, North Carolina’s first Twitter chat for teachers. She is passionate about innovative practices, instructional technology, student ownership of learning, and supporting teachers. Heather serves on the advisory board for the North Carolina Digital Learning Plan, North Carolina School for the Deaf and Catawba Science Center. She is a recipient of the 2015 NCMLE Central Office Administrator to Watch Award. She also received the 2016 Don Chalker Award for Excellence in Educational Leadership. Heather is the proud mother of one son, Jackson.
